What kniedzw linked was it. There isn't "etiquette" in the Victorian sense, but there are cultural assumptions about how you'll behave, and they're surprisingly foreign to Americans. Partly, I'd say, because of the way a pub looks like a collision between a bar and a restaurant -- that's where the confusion about where to order comes from.
